How do I submit a referral?
We only accept referrals from veterinarians.
Complete the online referral form by clicking HERE.
Upload patient's records and any imaging related to the case.
DICOM files can be shared directly by email to avidWV@gmail.com
We require a full physical exam within the last 6 months of the referral submission.
Blood work, urinalysis, and any other preoperative testing and medical records should be from within the last 4 weeks of referral submission.
AVID will review the client/pet information and documents and reach out directly to the owner via email.
Pricing:
All referring partners are sent updated pricing lists.
You may email avidWV@gmail.com for additional copies.
Pricing estimates may vary by patient based on size, species, imaging-study and time involved.
Pet owners will be given an estimate when they are contacted about scheduling their appointment.
Appointment Scheduling:
We will contact the owner BY EMAIL within 24-72 business hours to schedule their appointment and provide an estimate of services.
We will review any pre-visit appointment instructions and reply to any questions regarding the process.
Patients are admitted in the morning and discharged when ready to go home. Owners will be notified when their pet is ready to leave and given an estimated time at check-in.
Imaging is performed Monday-Friday only.
All pets will need to be food-fasted overnight.
Access to water is ok.
Referring Doctor’s Role:
Your role is to familiarize your client with AVID’s workflow listed above.
You may be asked to prescribe premedication for patients to reduce anxiety and overall anesthesia needed while the patient is with us.
Client follow up is the responsibility of the referring veterinarian.
This includes study results, treatment plans or additional procedures.
AVID does NOT discuss results with your client.
If the client contacts us, we will refer them back to you.
Reports and Results:
All results will be automatically sent to the email you submitted with the patient's referral.
Results should arrive within 72 business hours, unless sampling is sent to a diagnostic lab, and those reports may take longer to arrive.
